Forms and applications
Extra-Curricular Participation Packet Includes Code of Conduct Form, Emergency Procedures, Proof of Insurance Form, Student Driver/Rider Permission Form.
Facilities & Equipment Use Form (All three forms must be completed)
- Please submit at least 2 weeks in advance of your event date
- Facility Use Liability Release Form
- Facility Use Liability Release and Hold Harmless Form Use this form to request the use of any of our school buildings, parking lot or equipment.
Financial Aid Request for Student Activity Fee If you qualify for free or reduced lunch, or are experiencing exceptional adverse circumstances, you can request assistance with student activity fees by completing a financial aid request form.

Fundraising Planning Form Use this form to coordinate fundraising efforts among all constituents and reduce the possibility of both calendar and resource conflicts in the District. Medication Authorization Must be completed to authorize our staff to administer student medication.
Parking Information and Application for 2024-25 Complete this form to apply for parking at RHS
Prearranged Absence Form Complete this form if students will be absent for an extended period.
School Supplies List Suggested items for RHS students to bring to class and school in the fall.
Self-Medication Agreement Must be completed so that students may self-administer prescription and nonprescription medication.
Student Forms
Honors Contract Must be signed and on file by end of the first two weeks of each trimester.
Schedule Change Request Form Click to request changes to your class schedule, during the specified time period.
*New practices concerning scheduling choices and changes: While most schedule change requests must be made by the end of the first full week of each trimester, seniors must commit to their annual schedules by the last five school days in October. Learn about schedule change guidelines and deadlines, what happens after a class is dropped, other scheduling clarifications and required classes for senior year.
Service Learning Record Sheet Complete this form to claim your 90 required service hours as you accrue them.

Bullying Policy Harassment, intimidation or bullying and acts of cyberbullying by students, staff and third parties toward students is strictly prohibited. If you need to report bullying, please use the reporting form found in this section.
Plagiarism Policy Plagiarism is not tolerated. Learn what offenses are included in the policy and the related consequences.
Technology
Chromebook Contract Chromebooks are available in the library for students to check out on a daily basis. They must be returned at the end of the school day. Students must have a current signed contract on file with the library each year.
Student Acceptable Use Agreement and Policy This Student Acceptable Use Agreement is intended to promote responsible use and protect students and the school from liability resulting from the misuse of District-issued devices or District provided web-based services.
Photo & Social Media Opt Out Form If you would like your student to not be photographed or appear on our social media pages, please complete and return this form (Due the first month of school).

Social Media Application for Riverdale Clubs & Sports If students in clubs/organizations would like to create an “official” Riverdale social media account, they must fill out an application here.
